How to Get a Copy of Your Military Health Record

Discharged members of the military can find their personnel records with the National Archives and Records Administration (NARA). In 1992, the practice of filing medical records within a personnel file was discontinued. At this time, the army began filing discharged members' files with the Department of Veterans Affairs (VA). Subsequently, all other sections of the military have followed suit and now file with the VA as well. Active duty files can be found through the VA by calling (800) 827-1000.

Visit the National Archives website at www.archives.gov to request a personnel record. To obtain military records, you must be either a veteran or the next of kin to a veteran.

Access the online request form for military records. Within the section "Veterans and their families", there is a page called "eVetRecs" that allows you to fill out the form online, print and fax it. Click on the red "Request Military Records" at the bottom of the page to start the process.

Complete the four steps of request process. In step two, specify "Medical" under number seven when prompted about which type of record you would like. The form uses a pull-down menu to select the type of file you are looking for. Complete the remaining steps. You will also need the following information: veteran's complete name used while in service, his/her service number, social security number, branch of service, dates of service, date and place of birth.

Print the filled-out form. Once you have completed the entire form, print it out from your home computer. Check through the document to make sure all the information is correct.

Sign and date the form. If you are the next of kin, you must sign as well.

Mail or fax the form within 20 days. If the veteran is deceased, you will need to include proof of death, such as a copy of the death certificate, letter from a funeral home, or obituary. The address and fax number can be found on the form as well as the website. This service is free.
